If this application is successful I agree on behalf of the Applicant to participate in an evaluation process, to include completion of a brief progress report form, both 6 and 12 months after the award is made.

If this application is successful I also agree on behalf of the Applicant to participate in appropriate and relevant publicity, if required to do so by AYM.

For use in our reply to you by late May 2010 Please check the postage for the package is correct at the time of posting: Check the weight and size of your package and affix sufficient postage as AYM cannot accept packages with postage to pay.

3. Ensure the young person's Music Teacher posts their reference form direct to AYM

Hand this form to the relevant music teacher well in advance for their completion and ask them to post it direct to AYM by Friday 26 March 2010 at the latest.
